Description

Hyaluronic acid is a disaccharide present in all human connective tissues. It is one of the fundamental constituents of the extracellular matrix of the dermis. Thanks to its chemical structure, it increases the level of hydration of the connective tissues. It playes an essential role in preventing and slowing down natural skin aging, as it is essential for the formation of the collagen matrix and elastic fibers and for maintaining hydration.

It’s chemically identical in all species and in all types of tissue, which is why it has a total biocompatibility as it exists naturally in all living organisms, with an identical structure in both bacteria, animals or humans.

The use of hyaluronic acid is well documented not only in medicine and cosmetic surgery but also in other areas of medicine such as ophthalmic surgery and orthopedics. It’s used in aesthetic medicine is the result of complex laboratory syntheses obtained with the aid of biotechnologies.

There are 2 large families:

  • Hyaluronic acid of avian origin (type Hylaform).
  • Hyaluronic acid of bacterial origin Streptococcus equi also called NASHA (Non-Animal Stabilized Hyaluronic Acid) (type Restylane or Juvederm).

The  one that’s used in aesthetic medicine it is a sterile, viscoelastic, colorless and transparent gel composed of crossed molecules of hyaluronate. The quality and duration of reabsorption times depend on:

  • Quantity of the hyaluronate molecules
  • The intermolecular bonds present in 1 ml of product.

Indications

The cosmetic properties of hyaluronic acid are essential for restoring an optimal level of hydration, regenerating the skin’s hydrolipidic barrier, stimulating proper collagen synthesis, limiting inflammatory phenomena, making our skin smooth and compact once again.

Thanks to its anti-aging action, hyaluronic acid has earned the title of “fountain of youth” placing itself in first place among the most requested cosmetic and dermo-cosmetic ingredients used in anti-wrinkle treatments thanks to its undisputed super moisturizing, plumping and stimulating activity on the functionality of fibroblasts.

It has acquired an increasingly important role in the injectable and non-invasive treatments of Aesthetic Medicine, especially in the last decade. In fact, recently its use for aesthetic purposes has grown considerably, so much so that in the last 20 years, more than 30 million people have been successfully treated with hyaluronic acid-based products.

In Aesthetic Medicine, it is used for its filling and moisturizing function that allows both to fill the wrinkles of the face and to increase the volumes of certain areas of the face or body.

In addition, it is characterized by scarring and anti-inflammatory properties, for which it is used in other medical areas such as Rehabilitation and Sports Medicine, as a basis for infiltrative therapies of knee or hip arthrosis and joints in general.

Duration

Small injections are performed directly on the area to be treated, generally wrinkles and furrows on the face, but also lips, nose, cheekbones and chin. The duration of the treatment is 10-15 minutes and its effect is immediate, with results that last from 2 to 18 months, depending on the affected area, the product used and the physiological characteristics of the patient.

What must be done before the treatment?
L’uso di questo tipo di prodotti con fini estetici non ha bisogno del ricorso alla chirurgia e al bisturi. Questo significa che, generalmente, non vengono richiesti al paziente esami preliminari come analisi del sangue o delle urine. In ogni caso bisogna comunque considerare che si tratta di un prodotto che va iniettato nel corpo, quindi anche se si tratta di una delle procedure più sicure della Medicina Estetica, è sempre di fondamentale importanza informare il medico sul proprio stato di salute generale, su quali farmaci si hanno assunto nelle settimane precedenti, su eventuali gravidanze, reazioni allergiche all’anestesia, episodi di herpes e soprattutto se il paziente dovesse aver già realizzato trattamenti iniettabili dovrà specificare quali sostanze sono state utilizzate ed eventuali complicazioni o reazioni collaterali.

How often can the treatment be repeated?
I tempi possono variare, ma il trattamento può essere essere effettuato più volte senza particolari controindicazioni. In generale, si consiglia si può ripetere il filler per le rughe ogni 6-12 mesi e ogni 6 mesi quello per le labbra.
Can the treatment be done in the clinic?
Le iniezioni di acido ialuronico vengono effettuate in ambulatorio e il trattamento dura 15-30 minuti. Generalmente non è necessaria l’anestesia locale, ma talvolta può essere utilizzata una pomata anestetica soprattutto per il trattamento di zone più fastidiose come le labbra. Solo nel caso di trattamento con filler a lungo riassorbimento come quelli usati per il trattamento del mento o della regione degli zigomi, è consigliabile una anestesia locale o tronculare.
Does the treatment with hyaluronic acid injections leave a significant after-effect?
No, trattandosi di una sostanza naturale, l’acido ialuronico non presenta particolari controindicazioni. Nelle prime 24-48 ore dopo il trattamento a base di acido ialuronico può apparire un lieve rossore e la zona trattata potrebbe risultare leggermente più gonfia rispetto al risultato che si vuole ottenere. Raramente si può verificare un piccolo livido nella zona di iniezione. Dopo qualche ora è comunque possibile coprire la zona che è stata trattata con del trucco o del rossetto.
Are there any side effects or contraindications?
Essendo una sostanza naturale già presente nell’organismo umano, è considerato un prodotto sicuro, però questo non significa che non possano verificarsi effetti collaterali. Le linee guida dell’ISAPS raccomandano, pertanto, di non utilizzare con troppa leggerezza i filler a base di HA, perché anche se possono sembrare totalmente sicuri, si tratta sempre di sostanze che vengono iniettate nel corpo e che in caso di reazioni avverse sono difficili da rimuovere e potrebbero richiedere un intervento chirurgico. Le complicanze o effetti collaterali che potrebbero insorgere includono ematomi, gonfiori e granulomi in corrispondenza della zona trattata che possono provocare anche irregolarità e asimmetrie. Potrebbero prodursi anche reazioni allergiche al prodotto, rossori e infezioni. L’applicazione di ghiaccio dopo il trattamento può aiutare a facilitare la minimizzazione dell’effetto di queste reazioni. Altre possibili problematiche rientrano nel novero delle complicanze previste a livello medico ma estremamente rare.
In case of side effects, is there a solution?
La Ialuronidasi è un enzima che degrada l’acido ialuronico, permettendone il riassorbimento. Viene utilizzata per perfezionare o correggere gli effetti di un filler, ma la procedura deve essere eseguita con precisione e cautela da un medico specialista. È una procedura che viene realizzata nei casi in cui l’infiltrazione provocasse complicazioni, come per esempio asimmetrie o granuli, che possono compromettere il risultato estetico del trattamento. In questi casi è possibile iniettare la ialuronidasi nella zona interessata per neutralizzare il prodotto infiltrato ed eliminarne l’effetto. Il risultato è praticamente immediato, perché la sostanza agisce entro un’ora e i risultati sono visibili nel giro di 24 ore.
What is better: botox or hyaluronic acid?
Entrambi possono avere buoni risultati, ma è necessario effettuare un’anamnesi accurata e capire, durante la visita, quale di questi trattamenti sia più indicato.
How do i know if i am allergic?
L’allergia all’acido ialuronico è estremamente rara se non quasi inesistente, considerando che è un fisiologico costituente del tessuto connettivo. In ogni caso si può fare un test da un Allergologo o da un Immunologo dotato di tutto ciò che serve per la terapia medica d’urgenza.

Sono diverse le aziende che producono ottimi prodotti, l’aspetto fondamentale da valutare non è tanto la marca quanto il tipo di correzione di cui il paziente ha necessità (labbra, zigomi, ridefinizione del profilo, codice a barre ecc…) e quindi scegliere il prodotto giusto. Is the duration of the effects the same for everyone?
La durata varia a seconda del tipo di acido ialuronico usato e anche a seconda di vari fattori individuali come età, fototipo, tipo di cute, stile di vita (come fumo, alimentazione, attività fisica ecc)
Does hyaluronic acid stabilize after various treatments?
Se fatto con regolarità i risultati permangono più a lungo e più stabili nel tempo . In sintesi vanno quasi a combaciare con quello che viene definita durata biologica e durata estetica
Why was hyaluronic acid for breast augmentation banned?
È sconsigliato unicamente perchè, essendo radiopaco, crea difficoltà nella lettura delle ecografie e mammografie.
How long should sun exposure be avoided?
Se non compaiono lividi, si consiglia di non esporsi al sole per circa una settimana, se compaiono, il tempo si allunga fino a 3 settimane. Comunque è raccomandabile utilizzare una protezione 50* o schermo totale nel caso in cui si prevede di esporsi al sole
Can it be done while breastfeeding?
Non c’è nessuna controindicazione al trattamento perchè l’acido ialuronico non è un farmaco, non passa nel sangue e nel latte
